Most likely that is a commercial recipe from some large bakery or corporation. They most likely invented the recipe and it may be hard or even impossible to find, even here.
How large is the loaf?
How much does it weigh?
What is the name brand? Is it multi grain, whole wheat, white or what?
Sounds like the topping is a litle bit of Streusel or it could be made from oat bran, butter(?) and sugar.
If it has apples in it were they dried apples? (most likely, dried, . fresh apple pieces would make the bread too soggy)
If you read the label and ingredients real carefully it should not be too hard foryou to make a bread very similar to that by experimenting a little bit.
